What does is_confidential mean?

Setting is_confidential=1 hides the seller's name and domain from the public file. The spec allows this for a minority of sellers.

Do I need sellers.json as a publisher?

No — publishers use ads.txt. sellers.json is for exchanges, SSPs, and intermediaries.
